Camper Forms

There are several forms that Independent Lake Camp requires campers to fill out and return prior to attendance.  The forms are all available to view and download within your camper account.  You may also view and download some of the new camper forms and policy information below. Simply log in to your account and then follow the instructions in the right column.

Form Instructions

  1. After reading these instructions, Login to your camper account:

  2. To log in, type in your email address and password
  3. If you do not remember your password click on the “Retrieve / Set Password” Box on the lower left of the log in page, type in your email, and a link will be emailed to you to retrieve or set your password
  4. If you have never logged in before click on the “Retrieve / Set Password” Box on the lower left of the log in page, type in your email you used in our previous system, and a link will be emailed to you to set your password
  5. Once logged in, click on the “Forms & Documents” link beneath the Forms Dashboard
  6. You will see the following documents that are only there to provide information, please read through them to assist you in completing your forms:
  • 2016 Camper Account Information
  • 2016 Lazy Day Information
  • 2016 Suggested Packing List and Cell Phone Policy
  • 2016 Transportation Information
  • ILC Tipping Policy
  • Medications Brought to Camp
  1. Below them, you will see the following 6 mandatory forms to be completed online:
  • Additional Options and Camper Account Deposits Form
  • Bunk Request Form
  • Camper Account Parental Controls Form
  • Camper Profile Form
  • Medical Form
  • Transportation Form

NOTE: You will also see a “Camper Application” which you already completed when you registered (even if you registered through the old system, we transferred over that information)

  1. Click on the forms and start filling them out! Once a form is completed, it will change from being listed in black to being listed in green.
